Balance Medical Center has uploaded its Patient Intake Forms to help you get started in the Program.
Please select the appropriate patient form from the list, download and print a copy. It is important that you complete this form to the best of your ability before your scheduled consultation so that you and the doctor can accomplish all that is required in the 60-minute appointment. It is also important that you complete all questions on the questionnaire to help the doctor become familiar with your medical history.
